Job summary

Community Bridges, Inc. (CBI) is seeking a Lead Navigator to work with individuals experiencing homelessness, coordinating and monitoring access to behavioral health and medical services in Arizona.

The Lead Navigator assists with outreach, engagement, and wrap-around services, mentors Navigator II, and coordinates housing referrals along the discharge process as part of a multidisciplinary team.

Qualifications

• Highschool diploma or GED required. Associates Degree (or higher) in a field related to behavioral health preferred but not required.
• 1-3 years of full-time health care related work experience with exposure to behavioral and medical crisis situations preferred but not required.
• Or equivalent combination of education and experience.
• Current AZ Driver's License.
• Clear 39-month Motor Vehicle Record.
• Behavioral Health Technician (BHT) per A.A.C. R9-10-101.33 and CBI policies.
• Arizona Fingerprint Clearance card, site specific.

Responsibilities

• Conduct outreach and engagement with homeless individuals to assist in ending homelessness.
• Coordinate and monitor RBHA and non-RBHA community-based services for clients.
• Provide case management and wrap-around supportive services.
• Mentor Navigator II to onboard and guide program requirements.
• Coordinate entry, housing referrals, leasing up, wrap around services, and discharge.
