Job summary

The Salvation Army USA Central Territory is seeking a dedicated Case Manager to support low-income clients on their path to family sufficiency. You will assess needs, develop case plans, coordinate referrals, and document progress with accuracy.

In this role, you will maintain confidential client records, build relationships with community partners, and ensure compliance with ethics and Safe From Harm training. A bachelor’s degree and 2+ years of experience are preferred.

Qualifications

  • Bachelor's degree or equivalent combination of training and experience.
  • 2+ years of case management or applicant interview experience.
  • Experience with program target populations preferred.
  • Must complete Safe From Harm training.

Responsibilities

  • Facilitates case management within program guidelines; develops rapport and case plans; tracks progress and coordinates referrals.
  • Maintains relationships with community agencies; promotes services and ensures releases of information on file.
  • Creates case plans and helps participants utilize agency resources and outside referrals.
  • Enhances knowledge through community and agency educational forums.
  • Uses electronic database to enter and maintain case records and monitor funds and resources.
  • Fulfills surveys and provides information for administrative reports.
  • Performs other duties as assigned.
