Job summary

Interfaith Works in Rockville, MD is seeking a dedicated Case Manager to support program participants in securing stable housing and accessing essential services. The role emphasizes strengths-based, trauma-informed care, and Housing First principles, with a focus on coordinated service delivery.

The case manager will oversee ~25 participants, conduct weekly meetings, and collaborate with housing and health resources to promote lasting stability.

Qualifications

• Bachelor's degree or equivalent experience in behavioral health or human-services field.
• Experience in case management, housing navigation, homeless services, crisis intervention, or community-based services preferred.

Responsibilities

• Establish professional relationships with participants using strengths-based, trauma-informed, and Housing First approaches.
• Maintain a caseload of approximately 25 participants.
• Meet with participants at least weekly to assess housing needs and plan.
• Develop and update individualized service and housing plans with participants.
• Assist with housing applications, documentation, and access to community resources.
• Coordinate services with providers, landlords, and government agencies.
• Participate in Coordinated Entry meetings and interdisciplinary team meetings.
• Maintain accurate HMIS records and program documentation.
• Provide crisis intervention and safety planning.
• Attend required trainings and perform other duties as assigned.
