Qualifications

• Bachelor's degree in human services or a related field.
• Foreign language fluency in Arabic or Spanish in addition to English.
• 1+ years of case management or direct services experience.
• Strong written and oral communication skills.
• Ability to prioritize competing priorities and make sound judgments.
• Ability to complete tasks while navigating frequent interruptions.
• Ability to deliver creative, resourceful solutions to unique challenges.
• Strong organizational and time management skills.
• Valid Driver's License and reliable transportation.
• Ability to drive a 15-passenger van.
• Willingness to work evenings and weekends when needed.

Responsibilities

• Develop housing-first focused service plans tailored to the client's housing needs and guide families through the housing location process.
• Proactively and creatively seek new housing opportunities and resources in Fairfax and nearby areas to assist families with locating housing.
• Address safety issues and refocus families on their housing plan when needed.
• Plan, schedule, coordinate, and conduct Family Shelter Planning Team meetings or professionals' meetings to assist in housing planning.
• Once housed, outline supports needed to address housing barriers and to maintain stable housing.
• Assist clients in securing services and coordinate delivery of services (employment, housing, finance).
• Assist clients in improving money management by developing a working budget using financial information provided by clients.
• Build and maintain relationships with area landlords who will work with families with multiple barriers.
• Educate clients on tenant rights, leases, and housing responsibilities.
• Assist with developing a rental assistance package and document fund distribution; lead recertification if needed.
• Function as part of a multidisciplinary team to ensure timely services.
• Disseminate relevant information to staff in a timely manner.
• Encourage clients to focus on Housing Plan objectives and sustainability.
• Assist clients in discharge planning.
• Timely documentation of client contacts in files and HMIS.
• Maintain complete client records per agency standards.
• Proficiently use Microsoft Suite in day-to-day work.
• Maintain knowledge of emergency procedures.
• Be in rotation for Intake Phone as directed by supervisor.
• Participate in staff meetings and trainings.
• Commit to continuous learning and applying best practices.
• Empower clients toward housing success and, if needed, utilize Shelter House vehicles for housing viewings.
• Ensure compliance with agency policies.
• Exemplify iC.A.R.E. values and perform other duties as assigned.
