Job summary

Shelter House in Fairfax County, VA is seeking a Supportive Housing Case Manager to oversee a caseload of 10-20 households within our supportive housing programs. You will use a trauma-informed approach to help clients achieve housing stability and self-sufficiency, coordinating services across health, housing, and social supports.

Qualifications

• Bachelor's degree in human services/related field or commensurate experience.
• 1+ years of case management/direct services/other related experience.
• Strong written and oral communication skills.
• Ability to prioritize competing priorities and make sound judgements.
• Ability to complete tasks while navigating frequent interruptions.
• Ability to deliver creative, resourceful solutions to unique challenges.
• Strong organizational and time management skills.
• Valid Driver’s License and reliable transportation.
• Ability to Drive a 15 passenger van.
• Willingness to work evenings and weekends when needed.

Responsibilities

• Perform assessments to gauge clients' strengths, health, social and educational needs, and employment readiness.
• Develop Individualized Service Plans.
• Assist clients in securing services and coordinate delivery of all services to clients.
• Provide support to clients experiencing crises.
• Create and maintain financial budgets and money management education with clients.
• Encourage clients to achieve their objectives in their service plan.
• Empower and guide clients in their housing program completion.
• Assist clients in planning for discharge/graduation from the program.
• Maintain complete client records in accordance with agency best practice standards.
• Timely documentation of all contact with clients in HMIS and client file.
• Participate in on call rotation.
• Attend staff meetings and trainings as directed by supervisor.
• Ensure compliance with all agency policies.
• Collaborate with Shelter House Inc. and partner agencies to ensure timely services.
• Disseminate relevant information to appropriate staff.
• Utilize Microsoft Office in day-to-day work requirements.
• Safely operate Shelter House vehicles to drive clients.
