Access Services in Fort Washington, PA, seeks a compassionate Housing Case Manager to empower residents in Montgomery County on their path to stable housing and wellness. This full-time, on-site role blends mental health support with housing stability duties and liaises between clients, landlords, and community resources.

The position requires developing IRPs, coordinating with partners, and ensuring HUD compliance, with travel as needed and no remote work option.

Qualifications

• Ability to communicate well and possess strong interpersonal skills.
• Ability to direct and prioritize own work, work as part of a team, and be flexible.
• Knowledge of the mental health system, homelessness and the housing system.
• Skills in person-centered screening and assessment.
• Skills in conflict and crisis resolution.
• Proficiency in or willingness to learn workplace applications and technologies.

Responsibilities

• Meet with referred individuals with the Housing Locator and with those moving into housing.
• Develop and update Individual Rehabilitation Plans (IRPs) with the individual and team.
• Coordinate with landlords and community resources to bridge gaps in housing stability.
• Maintain organized documentation for move-in, recertification, and ongoing requirements.
• Conduct monthly home visits and provide support during transitions.
• Assist with transportation for health appointments, job interviews, and activities.
• Identify barriers to housing stability and develop creative solutions.
