Total Care Connections in Phoenix, AZ is seeking an Account Executive who thrives on building relationships with hospitals, physicians, and referral partners to expand our community-based care services. You’ll conduct client assessments, develop individualized care plans, and coordinate with Care Managers to ensure high-quality service.

Qualifications

• Experience in home care, healthcare, hospice, senior living, or community-based services.
• Knowledge of Arizona Medicaid HCBS programs.
• Experience with referral sources such as the VA, Banner, UnitedHealthcare, hospitals, SNFs, and community partners.
• Private-pay business development experience.
• Client assessment and care planning experience.

Responsibilities

• Building and nurturing relationships with referral partners across hospitals, SNFs, physicians, and pay sources.
• Conducting client assessments and meeting families to understand care needs.
• Developing personalized care plans that set clients up for success.
• Collaborating with Care Managers and Operations to ensure seamless service delivery.
• Serving as a trusted resource for clients, families, and referral partners.
• Representing Total Care Connections at networking events and in the Phoenix healthcare community.
