Job summary

Volunteers of America Colorado is seeking a dedicated Case Manager to direct and facilitate the delivery of support services for participant households. The role includes needs-based assessments, community outreach, and comprehensive case management with service coordination across programs.

The ideal candidate has a bachelor’s in human services or related field, at least one year of direct case management experience, and a Colorado license with mileage reimbursement available.

Qualifications

• Bachelor’s degree in human services, social work, or closely related field or related experience.
• 1 year of direct, full-time case management experience with field-relevant best practices.
• Colorado driver’s license and state-mandated automobile insurance.
• Personal vehicle for work-related travel; mileage reimbursement available.
• Complete agency and program credentialing within 12 weeks of hire.

Responsibilities

• Direct and facilitate delivery of appropriate support services for participant households.
• Conduct needs-based and program eligibility assessments and engage in outreach.
• Provide case management services and program-wide service coordination.
• Aim for demonstrable progress in applying field-relevant best practices.
