Volunteers of America Colorado is seeking a dedicated Case Manager to direct and facilitate the delivery of support services for participant households. The role includes needs-based assessments, community outreach, and comprehensive case management with service coordination across programs.
The ideal candidate has a bachelor’s in human services or related field, at least one year of direct case management experience, and a Colorado license with mileage reimbursement available.

**The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. Job duties include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination. The case Manager is charged with making demonstrable progress towards obtaining and applying knowledge and experience in field-relevant best practices and standards.
