Job summary

Volunteers of America Colorado is seeking a dedicated Case Manager for the Back Home SSVF program. The role centers on rapid rehousing and housing stabilization for veterans, focusing on housing case management and connecting clients with benefits, health referrals, and financial resources.

Ideal candidates have at least four years in human services, a Colorado driver’s license, and a personal vehicle for travel.

Qualifications

• Four years of direct experience in the provision of human services/related experience or education.
• Must possess a Colorado driver’s license and state-mandated automobile insurance.
• Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available).
• Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.

• The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities.
• Conducts needs-based and program eligibility assessments, engages in community outreach, provides case management services, and coordinates program-wide services.

PROGRAM

This position is for the Back Home SSVF (Supportive Services for Veteran Families) program which is a Rapid Rehousing program that utilizes a 90-day model to find and stabilize housing for veterans experiencing homelessness or at risk of homelessness. Primarily focusing on housing case management, but also offering wrap-around case management services assisting to connect veterans to Public Benefits, VA benefits, Health referrals, and Financial health referrals.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. Job duties include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination. Case Manager is charged with making demonstrable progress towards obtaining and applying knowledge and experience in field-relevant best practices and standards.
