Job summary

Volunteers of America Colorado seeks a dedicated Case Manager who directs and facilitates the delivery of supportive services to participant households. You will conduct needs-based assessments, engage in community outreach, provide case management, and help coordinate program-wide services to drive measurable progress.

The role requires a Colorado driver’s license, a personal vehicle for travel, and the ability to complete agency credentialing within 12 weeks of hire.

Qualifications

  • 1 year of direct, full-time case management experience required.
  • Bachelor’s degree in human services, social work, or closely related field.
  • Colorado driver’s license and state-mandated automobile insurance required.
  • Must own a personal vehicle for work-related travel (mileage reimbursement).
  • Credentialing within 12 weeks of hire and ongoing compliance.

**The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. Job duties include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination. The case Manager is charged with making demonstrable progress towards obtaining and applying knowledge and experience in field-relevant best practices and standards.

Qualifications of Position
  • Bachelor’s degree in human services, social work, or a closely related field or related experience.
  • 1 year of direct, full-time case management experience demonstrating the ability to consistently apply field-relevant best practices and standards.
  • Must possess a Colorado driver’s license and state-mandated automobile insurance.
  • Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available).
  • Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.

Position Type and Expected Hours of Work
  • Full-Time
  • Work hours vary depending upon program requirements but are typically scheduled around a 40-hour work week M-F.
Position Salary Range
  • $22.00 - $24.00
