You will conduct needs-based assessments, engage in community outreach, provide case management, and help coordinate program-wide services to drive measurable progress.

The role requires a Colorado driver’s license, a personal vehicle for travel, and the ability to complete agency credentialing within 12 weeks of hire.

Qualifications

• 1 year of direct, full-time case management experience required.
• Bachelor’s degree in human services, social work, or closely related field.
• Colorado driver’s license and state-mandated automobile insurance required.
• Must own a personal vehicle for work-related travel (mileage reimbursement).
• Credentialing within 12 weeks of hire and ongoing compliance.
