Job summary

3M's Services LLC is seeking a Qualified Professional to oversee community-based supports for individuals with IDD and mental health needs in North Carolina. The role emphasizes person-centered services, independence, and community involvement.

The QP will manage a caseload, collaborate with families, Care Managers, and staff, and ensure compliance with plans while promoting dignity and growth.

Qualifications

• Experience coordinating ISP/Plan of Care meetings.
• Ability to supervise Direct Support Professionals and coach staff.
• Strong communication with families, guardians, and care teams.

Responsibilities

• Manage an assigned caseload of individuals receiving community-based services.
• Coordinate and monitor services according to ISP/Plan of Care.
• Participate in ISP and treatment team meetings and planning meetings.
• Maintain regular communication with individuals, families/guardians, Care Managers, and leadership.
• Complete required member monitoring and site visits.
• Monitor goals, progress, and service utilization.
• Supervise and support Direct Support Professionals.
• Conduct staff observations and supervision visits.
• Train staff on goals, support needs, and documentation.
• Review service documentation for accuracy and timeliness.
• Monitor authorizations, consents, and assessments.
• Monitor EVV compliance where applicable.
• Identify barriers and coordinate resolutions with departments.
• Assist with interviewing.
