Job summary

Skill Creations, Inc. seeks a Qualified Professional (QP) to join its interdisciplinary team. You will guide person-centered plans and oversee program development for individuals served, coordinating services and ensuring outcomes align with goals. Strong communication with families and staff is essential.

The role requires experience with intellectual and/or developmental disabilities, a valid NC driver’s license, and flexibility to meet on-site needs and emergencies as they arise.

Qualifications

• Bachelor's degree in Human Services or closely related field with required experience.
• Master's degree with post-graduate experience preferred.
• Valid NC driver's license and ability to lift 50 lbs. and drive company vehicles.

Responsibilities

• Guide development of person-centered plans and programs for individuals.
• Coordinate and monitor services and supports to meet each individual’s goals.
• Serve as liaison between individuals, families/guardians, the agency and staff.
• Maintain and build relationships with families and regulatory personnel.
