Job summary

The North Carolina Department of Health and Human Services (DHHS) seeks a Human Services Provider Program Coordinator II to serve as the Qualified Intellectual Disabilities Professional (QIDP) in habilitation services for residents at the J. Iverson Riddle Developmental Center.

You will lead development of Person Centered Plans and supervise staff across three shifts. This role requires supervisory experience and deep knowledge of IDD, with responsibilities including transition planning and

Qualifications

• Bachelor’s degree in a human services or related field.
• Two years of experience providing professional or direct services to individuals with disabilities.
• Documented supervisory experience.
• Thorough knowledge of intellectual and developmental disabilities concepts.

Responsibilities

• Develop Person Centered Plans (PCP) for residents on caseload.
• Supervise implementation of the treatment plan across three shifts.
• Supervise the Home Manager and office staff.
• Assist in transition planning for community living arrangements.
