Job summary

Gulf Coast Center in Brazoria and Galveston Counties seeks an IDD Habilitation Coordinator to develop and monitor habilitation plans for residents in institutional settings and those transitioning to community living.

You will coordinate service planning, ensure regulatory compliance, maintain stakeholder communication, and document activities to support responsive, person-centered care and safe service delivery.

Qualifications

• Bachelor's degree in social, behavioral, health or human services.
• One year of experience with individuals who have IDD.
• Strong written and verbal communication and documentation skills.
• Willingness to travel within Brazoria and Galveston Counties.

Responsibilities

• Coordinate the service planning team to develop the Habilitation Service Plan using a person-centered process.
• Monitor compliance with HHSC, TAC standards and track client progress.
• Maintain regular contact with individuals and families about status and needs.
• Complete required programmatic and administrative documentation promptly.
• Support agency compliance and build community partnerships as needed.
