Job summary

Gulf Coast Center (GCC) seeks an IDD Service Coordinator to provide service coordination for adults and children with intellectual and developmental disabilities. You’ll enroll, assist families, develop person-directed plans, advocate for services, and coordinate care across Brazoria and Galveston Counties.

This role involves field visits and office work. Requirements include a Bachelor’s degree in Social Services and at least 1 year of IDD-related experience.

Qualifications

• Bachelor's degree in Social Services.
• Minimum one year experience working with individuals with IDD.

Responsibilities

• Complete initial Service Coordination Assessment and update as needed.
• Coordinate PDP development and implementation; facilitate planning meetings and revisions; identify resources.
• Monitor participant progress in relation to the PDP.
• Maintain documentation and billing within required timeframes.
• Travel within Brazoria and Galveston Counties as needed.
