Job summary

The Harris Center for Mental Health and IDD in Houston, TX seeks a Service Coordinator to empower individuals with IDD by coordinating medical, psychiatric, residential, and financial supports. You will guide person-directed planning, bridge consumers with providers, and develop comprehensive care plans tailored to each client’s unique goals.

Responsibilities include conducting assessments, quarterly monitoring, maintaining records, and ensuring timely documentation.

Qualifications

• Minimum of one year experience in the human services field (preferred).
• Experience working with persons with intellectual disabilities preferred.
• Strong written and verbal communication and organizational skills.
• Ability to work autonomously in a fast-paced environment.
• Cultural sensitivity when engaging with diverse consumers and families.
• Must maintain a valid Texas driver's license and insurable status.

Responsibilities

• Conduct comprehensive assessments following policy and guidelines.
• Monitor client welfare at least quarterly based on individual needs.
• Update consumer files and maintain data integrity.
• Document records timely and accurately according to timelines.
• Travel to consumer homes and work in field-based settings when needed.
