Job summary

Camino Real Community Services seeks a Habilitation Coordinator IDD to ensure individuals in nursing homes receive comprehensive, person-centered support. Responsibilities

  • Serve as the primary liaison between clients, service providers, healthcare professionals, and community resources.
  • Conduct comprehensive assessments to identify client strengths, preferences, and areas requiring support.
  • Maintain accurate and timely documentation of service plans, progress notes, and compliance reports.
  • Advocate for client rights and access to necessary services, including housing, employment, and healthcare.
  • Facilitate meetings and communication among stakeholders to resolve issues and promote coordinated care.
  • Stay informed about changes in policies, regulations, and best practices related to intellectual and developmental disabilities.

About the Role:

The Habilitation Coordinator IDD (Intellectual and Developmental Disabilities) plays a critical role in ensuring that individuals with intellectual and developmental disabilities residing in nursing homes receive comprehensive, person-centered support services tailored to their unique needs. This position involves coordinating care plans, facilitating access to community resources, and collaborating with healthcare providers, families, and support staff to promote the well-being and independence of clients. The Habilitation Coordinator acts as an advocate, ensuring compliance with regulatory requirements while fostering a supportive environment that enhances quality of life. Through ongoing assessment and communication, the coordinator monitors service delivery effectiveness and adjusts plans as necessary to meet evolving client goals. Ultimately, this role is pivotal in bridging gaps between clients and service systems to empower individuals to achieve their fullest potential within their communities. The caseload will be primarily concentrated in Atascosa and Frio counties. Must drive to nursing facilities using an agency vehicle.

Minimum Qualifications:
  • Bachelor’s degree in social work, psychology, human services, or a related field.
  • Strong knowledge of community resources and service delivery systems for IDD populations.
  • Excellent communication and interpersonal skills.
Preferred Qualifications:
  • Experience working with individuals with intellectual and developmental disabilities.
  • Experience with Medicaid waiver programs and state IDD service regulations.
  • Proficiency in electronic case management systems.
  • Bilingual abilities or experience working with diverse cultural populations.
  • Ability to manage multiple cases and maintain detailed records in compliance with regulatory requirements.
