Job summary

Gulf Coast Center in Brazoria and Galveston Counties seeks a Team Leader-IDD Authority Services to train and mentor Service Coordinators, oversee an assigned caseload, and ensure high-quality person-directed plans for adults and children with IDD.

You will travel for field visits, coordinate care, monitor progress, and ensure accurate documentation and billing while fostering diversity and compliant practices.

Qualifications

• Bachelor's or advanced degree in a social, behavioral, or human service field including psychology, social work, nursing, education, or related.
• Minimum two years experience working with individuals with IDD.
• Requires Texas Driver's License, reliable transportation, and auto liability insurance.

Responsibilities

• Complete initial Service Coordination Assessment and update as needed.
• Coordinate PDP development and implementation; facilitate planning meetings and revisions; develop IPC/Service Option; refer to community resources; advocate for services.
