Job summary

Heart of Texas Behavioral Health Network in Texas seeks a full-time Habilitation Coordinator to serve as the primary advocate for individuals with IDD in nursing facilities. You will conduct interviews, develop and coordinate service plans, and monitor progress while ensuring resources are used effectively.

Job description

This position is the primary advocacy position for consumers with IDD and related conditions who reside in Nursing Facilities. The individual in this position should be able to effectively interact with consumers, families, nursing facility staff and other service providers in the community and should understand the unique role of a Habilitation Coordinator working in a nursing facility. This position requires independent and objective judgment. The person in this position must work independently within specific guidelines; must be able to effectively develop a program plan that identifies specific goals and service needs that are geared toward more independent living and includes service justifications and methods to evaluate progress toward individual outcomes. The person in this position must communicate effectively verbally and through written documentation. Experience in a social service setting is preferred.

EXAMPLES OF WORK PERFORMED:
  • Conduct interviews with consumers, families and nursing facility staff to determine needs related to quality-of-life issues.
  • Develop and coordinate implementation of Individual Service Plans, including developing justifications for specific services within the plan.
  • Provide specific assistance related to accessing alternative resources to meet needs.
  • Provide monitoring of both consumers and service providers to ensure adequacy and quality of services being provided.
  • Provide training and education to individuals with IDD in the nursing facilities about resources available in the community to better inform them in decision making as to where to live. Serve as primary advocate for moving the individual into a community setting.
  • Completing documentation related to service provision; developing person directed plans, service plans, community living discharge plans when necessary, and daily/monthly/quarterly progress notes.
Experience:

One (1) years’ experience working directly with people with Intellectual and Developmental Disabilities is required.

Education:

Position requires a bachelor’s or master's degree from an accredited college or university with major coursework in social work, psychology or related human service discipline.

KNOWLEDGE, SKILLS, AND ABILITIES
  • Must be able to effectively interact with consumers, families and service providers to maintain an adequate plan for each consumer on the caseload.
  • Must be able to consistently and in a timely manner develop and maintain an accurate flow of documentation.
  • Must be flexible in schedule yet organized to keep up with demands of individual consumers and program requirements.
  • Must be able to objectively evaluate the effectiveness of the services provided, including Habilitation Coordination services.
  • Ability to learn about and implement trauma-informed care practices within the scope of work for the position.
  • Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures.
  • Physical capacity may include prolonged sitting, necessitating carrying supplies up to 30 pounds, driving vehicles to transport consumers and/or their families and attend meetings at various sites. Applicants must be able to successfully participate, perform, and complete the skills required for CPR, First Aid, and PMAB certification.
Salary:

$46,851-$55,144 Annual $22.5245-$26.5100 Hourly *Depending on education and experience.*

Mon-Fri 8am-5pm
