IDD Habilitation Coordinator

Job summary

Paycom seeks a Habilitation Coordinator to support individuals with intellectual and developmental disabilities in a Nursing Facility setting in Wichita Falls, TX. You’ll monitor progress on individualized plans, lead planning meetings, and coordinate transitions with the broader care team.

The role requires frequent travel, strong documentation, and the ability to work with a diverse care network, including LARs and facility staff. Bachelor’s or higher in a related field is required.

Qualifications

• Bachelor’s or advanced degree in a social, behavioral, or human service field (psychology, social work, counseling, sociology, human development, etc.).
• At least 1 year direct work with individuals with IDD to be a QIDP.
• At least 2 years’ experience providing service coordination (ECC duties).
• Valid driver’s license and insurability; may require use of personal vehicle.
• Pass all required trainings.
• Must be able to travel frequently for community-based services.

Responsibilities

• Meet with individuals monthly in Nursing Facility to monitor progress on HSP goals.
• Facilitate Service Planning Team (SPT) meetings; develop and review the HSP.
• Monitor risk factors, make referrals, and adjust services to support community participation.
• Coordinate transitions from Nursing Facility to community settings with ECC and COCC.
• Enter and verify assessments using TMHP and PASRR processes as needed.
