Job summary

Camino Real Community Services seeks a Habilitation Coordinator IDD to ensure individuals in nursing homes receive comprehensive, person-centered support. The role requires a bachelor’s degree in social work, psychology, or a related field, strong knowledge of IDD services, and excellent communication and organizational skills.

Qualifications

• Strong knowledge of community resources and IDD service delivery systems.
• Excellent communication and interpersonal skills.

Responsibilities

• Develop, implement, and regularly update individualized service plans in collaboration with clients, families, and multidisciplinary teams.
• Coordinate and monitor the delivery of support services, ensuring they align with client needs and regulatory standards.
• Serve as the primary liaison between clients, service providers, healthcare professionals, and community resources.
• Conduct comprehensive assessments to identify client strengths, preferences, and areas requiring support.
• Maintain accurate and timely documentation of service plans, progress notes, and compliance reports.
• Advocate for client rights and access to necessary services, including housing, employment, and healthcare.
• Facilitate meetings and communication among stakeholders to resolve issues and promote coordinated care.
• Stay informed about changes in policies, regulations, and best practices related to intellectual and developmental disabilities.
