Job summary

Help at Home is seeking a Client Service Supervisor to support our clients with I/DD, developing PCISP with the client and team, coordinating caregiver schedules, maintaining records, and communicating with the client’s team.

No on-call or 24-hour availability. Office hours are 8:30AM–5:00PM, with hours varying to meet client needs. Weekly pay and opportunities for training and advancement.

Qualifications

• Minimum 18 years of age.
• High school diploma or equivalent preferred.
• Current PPD or Chest x-ray if applicable.
• Medicaid/Home Healthcare experience preferred.

Responsibilities

• Build and maintain relationships with clients, families, caregivers, and team members through regular check-ins.
• Use a relationship-focused approach to match clients and caregivers thoughtfully.
• Provide high-quality training to caregivers on the client's PCISP.
• Create and document client schedules based on needs, caregiver availability, and PCISP budget.
• Attend quarterly meetings to review PCISP and implement support strategies.
• Maintain records in EMR and prepare quarterly/annual reports and risk plans.
• Grow by admitting new clients to meet regional goals.
• Create great days and meaningful moments through relationship-focused care.
