Job summary

Independent Home Health is seeking a Care Coordinator for ID services to manage a caseload of about 45 individuals across Luzerne, Wyoming, and Lackawanna Counties, reporting to Towanda office.

You will coordinate care, match clients with staff, monitor ISPs per PA ODP waivers, and maintain accurate electronic records, traveling as needed.

This role requires 1–3 years of CM experience and a BS in Social Work or related field; strong regulatory knowledge and excellent communication are essential.

Qualifications

• 1–3 years of direct case management/service coordination in human services.
• Experience working with ID/DD populations.
• Valid driver’s license and reliable vehicle; willingness to travel across Luzerne, Wyoming, and Lackawanna Counties.
• Knowledge of PA ODP waiver programs and person-centered compliance.
• Bachelor’s degree in Social Work, Psychology, Human Services, or equivalent.

Responsibilities

• Oversee a caseload of around 45 individuals across Luzerne, Wyoming, and Lackawanna Counties.
• Coordinate care with direct care staff and client families.
• Match clients with appropriate staff based on needs and scheduling.
• Plan, implement, and monitor ISPs in compliance with PA ODP waivers.
• Report to Towanda Office Manager/ID Services Director with case evaluations and compliance reporting.
• Maintain electronic case records and documentation per PA regulations and agency standards.
