Job summary

My Independence at Home seeks a Case Manager to oversee all aspects of each case and build strong relationships with Participants in Philadelphia and surrounding counties. The role requires proactive engagement, coordinating care plans, and ensuring continuity of services across teams.

You will interact with Home Health Aides and CNAs, attend Individual Support Plan meetings, and ensure timely recertifications and annual surveys.

Qualifications

• Introductory meetings within 24 hours of enrollment.
• Execute Participant/Individual consent form.
• Complete Direct Care Worker Assignment Sheet.
• Conduct monthly check-ins to identify issues or concerns.

Responsibilities

• Manage all aspects of the case and foster relationships with Participants in Philadelphia and surrounding counties.
• Serve as liaison between Participant/Individual and Service Coordinator regarding service changes/issues.
