Job summary

Public Partnerships LLC in New Jersey seeks a Field-Based Support Coordinator to educate and assist Medicaid participants in self-directed care programs. You will visit participants in their homes, help manage care plans, and support hiring direct care providers for safe, independent living.

Expect to travel to participant homes (up to 60 miles) and coordinate with a field team from a home office. Proficiency with Microsoft Office, CRM platforms, and electronic records is required.

Qualifications

• Based in Hudson County, NJ
• Preferred degree in social work, psychology, or related field
• 1+ years experience with vulnerable populations
• Valid driver’s license with reliable vehicle
• Proficient with Microsoft Office and digital tools

Responsibilities

• Visit participants at home (up to 60 miles) to provide guidance and education
• Assess home environments and address safety concerns
• Help participants understand and manage employer responsibilities
• Organize and manage caseload efficiently
• Collaborate with team while working independently in the field
