Job summary

Independent Home Health is seeking a dedicated Care Coordinator to manage a caseload of about 45 individuals with ID/DD across Luzerne, Wyoming, and Lackawanna Counties. The role involves field work, remote coordination, and reporting to the Towanda Office Manager / ID Services Director.

You will match clients with direct care staff, implement ISPs per PA ODP waivers, monitor progress, and ensure meticulous documentation while liaising with county services and families.

Qualifications

• Minimum 1–3 years of direct case management/service coordination in human services or home health.
• Experience with Intellectual and Developmental Disabilities (ID/DD).
• Valid driver’s license and willingness to travel across Luzerne, Wyoming, and Lackawanna Counties.
• Knowledge of PA Office of Developmental Programs (ODP) systems and Medicaid waivers.
• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.

Responsibilities

• Oversee a caseload of ~45 individuals across Luzerne, Wyoming, and Lackawanna Counties with coordinated home health services.
• Match clients with direct care staff based on needs, skills, compatibility, and availability.
• Review and monitor ISPs in compliance with PA ODP waivers; ISPs are created externally.
• Report to Towanda Office Manager / ID Services Director with regular evaluations and updates.
• Coordinate with county entities, medical professionals, and families; conduct field visits.
• Maintain electronic case records, progress notes, and service authorizations per PA regulations.
