Job summary

The Arc Alliance in Pennsylvania is seeking a Supports Coordinator (Exempt Staff) to provide comprehensive case management for individuals with ID/DD across Montgomery County and Philadelphia.

You will monitor health and safety, coordinate services through Pennsylvania Medicaid waiver programs, and connect individuals and families to vital community resources, traveling to homes and community locations as needed.

Qualifications

• Bachelor’s degree from an accredited college or university
• Minimum of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social sciences

Responsibilities

• Conduct interviews and gather medical, educational, social, and vocational information
• Develop, implement, monitor, and update Individual Support Plans (ISPs)
• Ensure services are delivered as written in the ISP
• Conduct required face-to-face visits to monitor health and welfare
• Coordinate direct services and community resources
• Respond to emergency situations to ensure participant safety
• Facilitate annual ISP reviews and reassessments
• Maintain documentation in HCSIS
• Support individuals and families throughout the planning and service process
• Provide information on fair hearing rights and assist with requests
• Participate in community outreach and agency fundraising initiatives
• Complete all required trainings
