Job summary

Independent Home Health is seeking a Care Coordinator for ID/IDD services to manage a stable caseload of about 45 clients across Luzerne, Wyoming, and Lackawanna Counties. The role reports to the Towanda Office Manager / ID Services Director and serves as the primary liaison between clients, families, staff, and county funding systems.

The position requires 1–3 years of case management experience, ID/DD expertise, a valid driver’s license, and willingness to travel.

Qualifications

• 1–3 years of direct Case Management / Service Coordination experience.
• Proven ID/DD-specific experience.
• Valid driver’s license and willingness to travel across Luzerne, Wyoming, and Lackawanna Counties.
• Strong knowledge of Pennsylvania ODP and Medicaid waiver programs.
• Bachelor’s degree in Social Work, Psychology, Human Services, or a closely related field.

Responsibilities

• Oversee a caseload of around 45 individuals across Luzerne, Wyoming, and Lackawanna Counties, coordinating home health and community services.
• Match clients with direct care staff based on needs, skills, and scheduling.
• Plan implementation and ISP monitoring in compliance with Pennsylvania ODP waivers.
• Report to Towanda Office Manager / ID Services Director with regular evaluations and updates.
• Coordinate with county entities, medical professionals, and families; conduct field visits to monitor care.
• Maintain meticulous electronic case records and service authorizations per regulations.
