Job summary

Independent Home Health is seeking a Care Coordinator to manage a caseload of about 45 individuals with ID/ID/DD across Luzerne, Wyoming, and Lackawanna Counties, reporting to the Towanda Office Manager. The role involves field and remote coordination, linking families, staff, and county funding systems.

The position requires 1–3 years in case management within human services, a valid driver’s license, PA ODP waiver knowledge, and a bachelor’s degree in a related field.

Qualifications

• Minimum 1–3 years of direct Case Management / Service Coordination experience.
• Proven experience working with individuals with Intellectual and Developmental Disabilities (ID/DD).
• Must have a valid driver’s license, reliable vehicle, and willingness to travel across Luzerne, Wyoming, and Lackawanna Counties.

Responsibilities

• Oversee a caseload of around 45 individuals across Luzerne, Wyoming, and Lackawanna Counties.
• Match clients with appropriate direct care staff based on needs and availability.
• Plan, implement, and monitor ISPs per PA ODP waivers.
• Report to Towanda Office Manager / ID Services Director and provide regular updates and compliance reporting.
• Maintain electronic case records and service authorizations.
