Job summary

The Whole Child Mental Health & Housing Services is seeking a dedicated ICMS Case Manager to deliver intensive case management for families experiencing homelessness or at risk of homelessness in the Los Angeles area. You will help eligible children and their families access housing, health care, education, and community resources through coordinated referrals.

Qualifications

• Bachelor’s degree in social work, psychology, or other mental health related discipline.
• Minimum of three years of direct service experience in social services, preferably within housing, homelessness, behavioral health, or healthcare settings.
• Knowledge of Los Angeles Continuum of Care programs, Department of Health Services (DHS), HMIS, and CHAMP systems.

Responsibilities

• Maintain accurate, timely, and complete client records in CHAMP and HMIS to ensure data integrity and contract compliance.
• Develop and maintain effective working relationships with housing staff, Housing Authorities, service providers, and community partners.
• Conduct comprehensive assessments to identify family strengths, needs, barriers, and service goals.
• Provide guidance to parents and caregivers regarding child behavior management and family stressors.
