Sauk Prairie Healthcare, Inc. seeks a Transformation Program Manager to coordinate organization-wide strategy deployment, partnering with the Director to set long-term goals and drive lean improvements across departments.

You will coach interdisciplinary teams, champion continuous improvement, and serve as an internal consultant to frontline staff, empowering problem-solving and transformative care delivery in a hybrid work setting.

Qualifications

• Bachelor’s degree in Business, Healthcare Administration, Project Management, Organization Improvement, or related field.
• Five+ years delivering project management, strategy deployment, and continuous improvement results.
• Healthcare setting experience preferred.
• Certified Professional in Healthcare Quality is a plus.

Responsibilities

• Identify, scope, lead, coach, and evaluate multiple improvement projects using Lean and change leadership.
• Develop and maintain portfolio of strategy deployment projects.
• Provide education and mentorship in continuous improvement methods.
• Act as liaison to ensure timely, high-quality project deliverables across departments.
• Analyze data, define metrics, and report progress to leadership.
• Champion transformation and enable frontline staff to problem-solve and improve care delivery.
