Qualifications

• Must have current RN or LPN license without restrictions.
• 3 years of nursing experience with one year including triage (preferred).
• Associates Degree in Nursing (preferred).

Responsibilities

• Monitors and responds to triage calls in person and/or on the nurse phone line.
• Directs patients to ERs, urgent care, home care advice, or schedule with a physician during office hours.
• Determines urgency based on brief assessment and patient history.
• Uses computerized clinical decision-making and algorithms as a guide.
• Directs high-risk cases to ER immediately or arranges for ambulance.
• Provides appropriate home health advice when ER admission isn't needed.
• Ensures accurate notes in the Electronic Health Record.
• Arranges appointments and consults with physicians as needed.
• Serves as a clinical resource and role model for nursing staff.
• Prioritizes tasks and completes work promptly.
• Assists in chart reviews and data compilation for audits and reports.
• Assists in developing nursing policies and procedures.
