Duke Health Integrated Practice in Durham, NC is seeking a TRIAGE NURSE to support our Rheumatology team via telehealth. The position is based in Durham with a hybrid work option, focusing on nursing triage, education and care coordination for patients.

Qualified candidates have an accredited BSN/ADN, NC RN license, BLS, and at least 2 years of nursing experience with 1 year in med-surg. Strong communication and the ability to manage patient data remotely are essential.

Formación

• Graduation from an accredited BSN, ADN, or nursing diploma program.
• Minimum of 2 years nursing experience and 1 year med-surg experience.
• Current RN licensure in North Carolina; BLS required.
• Ability to perform telehealth and care coordination.

Responsabilidades

• Provide nursing triage, assessment and education to patients via telephone and secure messaging.
• Manage patient needs including medication management and post-procedure instructions.
• Coordinate care and serve as liaison to patients, families and providers.
• Document nursing actions and patient responses; support performance improvement activities.
• Delegate tasks to unlicensed care staff and participate in own professional development.
