Qualifications

• RN license in MN required
• MnCHOICES certification preferred or obtainable within 21 days
• Public Health Certification preferred
• 5+ years of experience in community-based services (beyond degree)
• Experience in home and community-based services/case management is a plus

Responsibilities

• Manage a specified caseload of members and coordinate delivery of approved medical and social services
• Educate on benefits including Medicare and Medicaid and serve as primary contact for members
• Partner with physicians, providers and county entities to discuss care plans and progress
• Perform other duties as assigned
