Job summary

Qualifications

• RN Diploma or Degree in Nursing required or Bachelor in Nursing with Public Health Certification + 1–2 years in community-based services.
• Bachelor's degree in Social Work with 3–5+ years in community-based services preferred.
• Active, unrestricted RN or SW license in MN; MNCAT training preferred or obtainable within 21 days.

Responsibilities

• Manage a caseload of members and create individualized care plans.
• Coordinate delivery of medical and social services with providers and counties.
• Educate members on Medicare/Medicaid benefits and navigate benefits.
• Communicate progress with physicians, providers, and county entities.
• Maintain thorough documentation and adapt plans as needed; perform other duties.
