Job summary

Medica, a nonprofit health plan serving Minnesota and beyond, seeks a Supervisor to lead a team of Care Coordinators and Engagement Coordinators delivering care coordination services to State Public Program members and other Medica members.

You will partner with the Clinical Care Manager to implement improvement opportunities, ensure regulatory and quality compliance, and support remote and onsite teams, with an onsite presence at Minnetonka, MN three days per week.

Qualifications

• RN or SW licensure in MN required with active status.
• Minimum 2 years in community-based services and 1–2+ years supervisory experience.
• 5+ years of clinical experience and ability to supervise remote staff.
• MnCHOICES certification preferred or obtainable within 21 days.

Responsibilities

This Supervisor will oversee a team of Care Coordinators and Engagement Coordinators who provide Care Coordination outreach and services to our unengaged member population.

Medica's Supervisor is responsible for managing teams of care coordinators and engagement coordinators for the provision of care coordination services for State Public Program and other Medica members. The majority of members will be from our Care Coordination Products (MSHO, MSC+, SNBC) within State Public Programs and the Government division.

In addition, this leader will partner with our Clinical Care Manager in the delivery and evaluation of Medica Care System care coordination services, implementing improvement opportunities, managing essential care coordination duties and oversight of regulatory and quality compliance requirements.

Required Qualifications

• RN Diploma or Degree in Nursing (Associates or Bachelors) + a minimum of 2 years' experience in community based services, OR
• Bachelor's degree in Social Work + a minimum of 1-2+ years of community based services
• 5+ years clinical experience
• 1-2+ years supervisory experience of professional staff, including employees who are home office or located remotely; informal leadership that has included coaching and mentorship will also be considered

Required Certifications/Licensure

• Active, unrestricted RN OR SW License in the state of MN Required
• MnCHOICES Certified Assessor Training (MNCAT) preferred or ability to obtain within 21 days of employment
• Public Health Certification preferred, however not required

This position is an Office role, which requires an employee to work onsite at our Minnetonka, MN office, on average, 3 days per week.

The full salary grade for this position is $70,200 - $120,400. While the full salary grade is provided, the typical hiring salary range for this role is expected to be between $70,200 - $105,315. Annual salary range placement will depend on a variety of factors including, but not limited to, education, work experience, applicable certifications and/or licensure, the position's scope and responsibility, internal pay equity and external market salary data.
