Job summary

Centene Corporation is seeking a hybrid RN Care Manager to assess and coordinate care for complex/high acuity adult members. The role involves home visits, coordinating LTSS needs, and self-scheduling from a home office in the Dallas area.

Requires 4–6 years of clinical nursing experience and an accredited nursing degree with RN licensure; BSN preferred. Mileage reimbursement and flexible schedules are provided in a comprehensive benefits package.

Qualifications

• Graduate from an Accredited School of Nursing.
• 4–6 years of related clinical nursing experience.
• RN licensure in the state; BSN preferred; RUG certification within 90 days.

Responsibilities

• Evaluate complex/high risk members and develop plans for best outcomes.
• Coordinate long-term care plans and identify providers and community resources.
• Collaborate with care teams to ensure person-centered care and services.
• Monitor care plans, track status, and revise plans as needed.
• Educate members and families on disease processes, care options, and benefits.
• Serve as liaison between members, families, physicians, and facilities.
• Provide home visits as needed and coordinate service authorizations.
