Job summary

Centene Corp. in Dallas, TX is seeking a Senior LTSS Service Care Manager to assess and coordinate care for high-acuity adult members, with home visits and a flexible schedule.

The role requires RN licensure, BSN preferred, and 4–6 years in clinical nursing with care management experience; 7+ years preferred. You will develop personalized care plans and collaborate with providers to ensure quality, cost-effective care.

Qualifications

• Graduate from an Accredited School of Nursing with 4–6 years of clinical nursing experience.
• Bachelor's degree in nursing (BSN) preferred.
• 7+ years of clinical RN Case Management or Care Coordination with adult members is preferred.
• RN licensure in state or compact state licensure; NP licensure allowed.
• Strong documentation, problem-solving, communication, and organizational skills.

Responsibilities

• Perform home visits and assess member needs and coordinate LTSS services.
• Develop and monitor long-term care/service plans with care teams and providers.
• Educate members and families on disease processes, options, and benefits.
• Coordinate with providers to revise care plans as needed and ensure quality care.
• Maintain and document all member information to ensure compliance with guidelines.
