Centene Corp. seeks an RN with extensive care management responsibilities to coordinate in‑home assessments in the South Dallas area.

The role combines clinical judgment with patient advocacy to develop individualized care plans and to educate members and families on available services and benefits. The position is hybrid, serving long-term care members while collaborating with the care management team to access providers, community resources, and ensure high-quality, person-centered care.

Qualifications

• RN license required or equivalent licensure
• Bachelor’s degree in Nursing preferred
• 4–6 years of related experience preferred

Responsibilities

• Performs care management duties to assess and coordinate care for high acuity populations
• Develops personalized care/service plans for long-term care members
• Educates members and families about services and benefit options
• Coordinates with providers and community resources to address member needs
• Monitors progress and revises care plans to meet member goals
• Acts as liaison between member, family, and care providers
