Job summary

Fairview Health Services, based at the University of Minnesota campus in Minneapolis, is seeking an RN Transplant Coordinator to support kidney and pancreas transplant recipients through their post-transplant journey. You will coordinate care, educate patients and families, and work with a multidisciplinary team on long-term management.

This full-time, exempt role offers a hybrid schedule with onsite two days and remote three days per week, sharing call responsibilities and a 40-hour workweek.

Qualifications

• RN licensure required (MN)
• Bachelor's or Associate degree in nursing required
• BLS certification required

Responsibilities

• Coordinate patient care in the post-transplant phases.
• Collaborate with interdisciplinary team and community partners.
• Act as a liaison for transplant physicians for medical management.
• Provide telephone triage and make care decisions based on assessments and labs.
• Maintain knowledge of current transplantation practices and protocols.
