Mayo Clinic is seeking a Transplant Coordinator Nurse to coordinate all aspects of care for transplant patients. The role involves collaboration with patients, families, and a multidisciplinary team to meet needs across the care continuum.

Qualified candidates will have an RN license, 1 year of RN experience (3 years preferred), and the ability to lead and educate patients and families. This role may involve flexible hours and hybrid work as needed by the unit.

Qualifications

• RN licensed in applicable state.
• 1 year RN experience required, 3 years preferred.
• Must be able to work flexible hours and adapt to unpredictable situations.
• Must obtain CCTC/CCTN within three years of hire.
• Maintain BLS certification.
• Exemption status Nonexempt.

Responsibilities

• Coordinate all aspects of care for the transplant patient population.
• Assess and plan in collaboration with patient, caregiver, and multidisciplinary team.
• Provide patient and family education on a consistent basis.
• Provide leadership through activities such as community involvement, committee work, and quality improvement.
• Follow ANA/UNOS/FACT guidelines for transplant practice.
