Job summary

Sanford Health seeks a Transplant Coordinator to join the Kidney Transplant Team. The RN will coordinate care for pre- and post-transplant patients, including education, workups, and activation, with clinic hours Monday–Friday 8am–5pm and on-call duties shared among coordinators.

Requirements include a Bachelor’s degree in nursing and an active RN license; Minnesota licensure within six months may be required. Day shift, 32–40 hours per week, with salary in the stated range.

Qualifications

• Bachelor's degree in nursing required.
• Active RN license in state of practice or multistate license.
• Experience coordinating transplant-related care is a plus.

Responsibilities

• Coordinate education, workups, and activation for renal transplantation.
• Manage a caseload of pre/post-transplant patients.
• Coordinate referrals and respond to roadblocks in the transplant workup process.
• Provide education to patients, families, and staff.
• Ensure compliance with governing regulations in the transplant program.
