This full-time, exempt role offers a hybrid schedule with onsite two days and remote three days per week, sharing call responsibilities and a 40-hour workweek.
Fairview is looking to hire an RN Transplant Coordinator to join our Post Transplant Kidney/Pancreas Team at the University of Minnesota campus in Minneapolis. This is an exciting opportunity for an RN who enjoys building long-term relationships with patients, coordinating complex care, and working as part of a highly collaborative transplant team.This is a full-time, 80 hours per pay period, benefit-eligible, exempt position offering a flexible hybrid schedule. Team members work onsite two days per week and from home three days per week. Wednesday is a designated onsite day, with the second onsite day rotating based on clinic coverage needs.
Monday–Friday, 8-hour shifts scheduled between 8:00 AM–5:00 PM
Three work-from-home days per week
No regularly scheduled weekend or holiday shifts
Rotating weekend and holiday triage call responsibilities
Call is shared across a large team, resulting in a low number of call shifts each year
As an RN Transplant Coordinator, you will play an important role in supporting kidney and pancreas transplant recipients throughout their post-transplant journey. You’ll partner closely with patients, families, physicians, advanced practice providers, and other members of the multidisciplinary transplant team to coordinate care, provide education, triage patient needs, and help patients successfully navigate life after transplant.

The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
