Job summary

Enlyte in Visalia, CA is seeking a Field Case Manager to coordinate treatment plans for injured workers and return them to work. This full-time, hybrid role requires travel to in-person appointments and collaboration with physicians, employers, and patients.

You will develop individualized plans, monitor progress, and ensure appropriate, cost-effective healthcare services while maintaining timely communications with customers and stakeholders. RN licensed professionals encouraged to apply.

Qualifications

• Associates or Bachelor’s Degree in Nursing or related field.
• 2+ years of clinical practice; workers’ compensation experience preferred.
• Active RN license in good standing; URAC-recognized case management certification (CCM/CDMS/CRC/CRRN/COHN etc.).
• Strong advocacy, independent work ability, and excellent written communication.
• Proficient with Excel, Word, and Outlook.

Responsibilities

• Manage field case loads and coordinate with physicians, employers, and injured workers.
• Develop and implement individualized treatment plans to return to work.
• Evaluate progress and adjust goals with stakeholders.
• Attend appointments in person as required.
• Ensure cost-effective, appropriate healthcare services.
