Job summary

Maximus Federal is seeking a Field Registered Nurse to conduct in-home and community-based assessments for the MassHealth LTSS program. You will develop person-centered care plans, educate members and caregivers, and advocate for vulnerable populations.

The role offers a Monday–Friday schedule with hybrid options and travel reimbursement. You will review medical and functional needs, coordinate with providers, and document findings in electronic records, ensuring high-quality, compliant care.

Qualifications

• Active RN license in Massachusetts.
• Minimum of two years post-graduate clinical experience as an RN.
• Experience working with vulnerable populations (behavioral health, I/DD).
• Experience conducting clinical or functional assessments in home/community settings.
• Knowledge of MassHealth LTSS and HCBS programs.
• Proficiency with Microsoft Office applications and basic electronic documentation.

Responsibilities

• Complete medical and/or behavioral health assessments in various settings per contract requirements.
• Conduct on-site, video, and/or telephonic assessments to determine service needs and eligibility.
• Interpret clinical guidelines to support accurate determinations of care levels.
• Travel to client locations to perform assessments (≥50% of workweek).
• Maintain compliance with HIPAA and company confidentiality policies.
• Document assessments in electronic systems and serve as a resource to the team.
