Pride Health is seeking a Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs in Brooklyn, NY.

The role combines field visits with remote care coordination, offering a balanced schedule and meaningful community impact. The RN must be licensed in New York and bring strong care planning and collaboration skills to the interdisciplinary team.

Qualifications

• Active, unrestricted RN license in New York.
• Associate’s degree in nursing required (BSN preferred).
• Minimum 3 years of RN experience.
• Experience in MLTC, home care, geriatrics, or case management preferred.
• Strong communication, organizational, and critical thinking skills.
• Ability to work independently in a hybrid environment.
• Bilingual skills are a plus.

Responsabilités

• Conduct in-home assessments to evaluate members’ clinical, functional, and social needs.
• Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams.
• Coordinate care across home care, hospital, rehabilitation, and long-term care settings.
• Complete approximately 6 home visits per week within Brooklyn.
• Monitor patient progress and adjust care plans to ensure outcomes.
• Support safe discharge planning and reduce hospital readmissions.
• Communicate with members, families, caregivers, and providers.
• Maintain accurate and timely documentation in compliance with guidelines.
