Qualifications

• Education: Associates or Bachelor’s Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred.
• Certifications, Licenses, Registrations: Active RN license required; URAC‑recognized case management certification preferred.
• Skills: Proficient with Excel, Word, and Outlook Email.
• Travel: Must have reliable transportation to attend in-person appointments.

Responsibilities

• Demonstrate knowledge, skills, and competency in the application of case management standards of practice.
• Collaborate with treating physicians/providers and utilize available resources to create and implement treatment plans.
• Interview disabled persons to assess recovery and occupational factors.
• Work with employers and physicians to modify job duties to facilitate early return to work.
• Independently manage workload and prioritize cases; attend appointments and prepare status updates as needed.
