Job summary

Enlyte is seeking a Field Case Manager (RN) in the Salinas, CA area to coordinate treatment plans for injured workers. This full-time, hybrid role requires travel to in-person appointments and collaboration with physicians, employers, and customers to ensure effective and cost-efficient care.

Responsibilities include applying case management standards, advising on treatment plans, interviewing patients, and modifying job duties for early return-to-work.

Qualifications

• Education: Associates Degree or Bachelor’s Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred. Workers’ compensation-related experience preferred.
• Certifications, Licenses, Registrations: Active RN license in good standing; URAC-recognized case management certification.

Responsibilities

• Demonstrate knowledge, skills, and competency in case management standards of practice.
• Advise on a treatment plan using knowledge of injuries, medications, comorbidities, and job duties.
• Interview disabled persons to assess recovery and determine occupational relevance.
• Collaborate with treating physicians/providers and utilize resources to create and implement treatment plans.
• Work with employers and physicians to modify job duties to facilitate early return to work.
• Evaluate and modify case goals based on patient improvement and treatment effectiveness.
• Independently manage workload, prioritizing cases and planning how to manage them effectively.
• Attend injured worker appointments when needed and prepare status updates for customers.
